Website and Service Terms
Hawkeye Consulting Services, LLC
Effective date: October 23, 2026
These terms explain the services offered through Hawkeye’s website and booking process. Please read the service description, price, intake consents and any terms presented at checkout before paying. Contact Hawkeye if anything is unclear. These terms do not replace a physician’s medical consent or OMMA’s application requirements.
Services and responsibility for decisions
Hawkeye offers full-service application assistance and physician-evaluation appointments. Full-service assistance includes the service components shown at checkout, including the specified OMMA fee, a physician evaluation, document and photo guidance, and assistance preparing and submitting the application when authorized. Evaluation-only service provides the physician evaluation and, if the physician issues one, the recommendation form so the patient can complete their own application.
An evaluation does not guarantee a recommendation, and application assistance does not guarantee approval or a particular processing time. The physician makes the medical decision; OMMA determines licensing eligibility and final document acceptance. Hawkeye helps patients address corrections within the selected service. Website information is general service information and does not replace advice from your treating clinician.
Prices and reduced-fee service
The price and included services shown at checkout apply to the option selected. Current virtual options are $200 for full-service assistance with the standard OMMA fee, $120 for full-service assistance with the reduced OMMA fee, and $65 for an evaluation-only appointment. Separate clinic offers may have different prices. Review the final total before paying.
The $120 option requires qualifying reduced-fee evidence, such as the accepted Medicare, Medicaid/SoonerCare or 100% disabled veteran documentation described during intake. Evidence is reviewed after booking. If you do not qualify, Hawkeye will work with you to arrange a refund or an additional $80 payment for standard-fee service. The extra $80 is not automatically charged without arranging it with you.
Accurate information and application authorization
Provide accurate, complete information and documents that belong to the person applying. Review your intake answers and notify us promptly about mistakes or changes. You may correct intake information; a correction can affect the service, fee or documents required. Staff may need additional information before an application can be prepared or submitted. Do not submit another person’s records unless you have authority to do so.
Minors and caregivers currently receive staff-assisted service. Contact Hawkeye for assistance instead of selecting an adult service that does not apply. Physician assignment is handled by Hawkeye; the current online process does not offer a patient choice of physician.
Appointments, payment and interrupted checkout
Available appointment times are shown during booking. Complete the required payment and confirmation steps, then finish intake, document uploads and consent. An offered time is not a confirmed appointment until the booking process confirms it. Keep the payment-return page open until confirmation.
If you have already paid and the confirmation or return page fails, contact Hawkeye. Do not start over or pay again. You may return in the same browser and device within the original return window, or ask staff for a personal completion link after they verify your matching record. The link lets you finish missing intake information and consent; documents already recorded as received are not requested again unless a replacement is needed. Hawkeye will review payment and booking records separately when confirmation is uncertain.
Changes, cancellations and refunds
Contact Hawkeye as soon as possible if you need to change or cancel an appointment, chose the wrong service, or want to request a refund. We will review the service purchased, payment status and work already completed and explain the applicable options. No new cancellation penalty or refund deadline is established by these terms. The reduced-fee correction process is described above. Any specific policy shown and accepted at checkout will also apply, subject to applicable law.
Documents, communications and privacy
Provide requested documents through the designated intake or staff-assisted process. We review uploads and will contact you if a replacement is needed. Final document acceptance is determined by OMMA. Ensure that the email and telephone number you provide can receive service messages, and contact us if you do not receive expected updates.
Our Privacy Policy explains how information is handled. Do not send passwords or sensitive documents through public comments or public links. Access to third-party services, including Square, Google, Dropbox, Quo and OMMA, may also be subject to those providers’ terms and policies.
Website availability, appropriate use and updates
Do not misuse the website, interfere with its security, upload unlawful material or attempt to access another person’s records. Service interruptions may occur. If booking or intake is unavailable, contact Hawkeye so we can help. An interruption should not lead you to make a duplicate payment.
We may update website information and these terms as our services change. We will identify the effective date of the current terms and provide additional notice when required. Changes will not be used to misrepresent the service or price already purchased. Nothing here removes rights that cannot be excluded under applicable law.
